What Information is Collected

Unless you complete a form using the website, Netcare does not collect or store personally identifiable information about you on the website. When you submit an online form, such as an employment application, we may ask you for your name, e-mail address, and some other pertinent information relevant to your contact request. This information is not accessible by anyone other than Netcare via our website. Personal Health Information (PHI) about Netcare clients is NOT collected or stored on the website. PHI is stored in an individual’s medical record. Medical records are not accessible through the website. Please see Netcare’s Notice of Privacy Practices for information on how and under what circumstances your PHI is used and/or disclosed.
