- If you have a client who is in crisis and will be coming to Netcare, call 614-276-CARE and give our ERS the following information:
- Client’s Name
- Client DOB
- Your Name
- Your Contact Phone Number
- Presenting issue and symptoms the client is experiencing
- Current MH/AOD treatment providers with whom the client is connected
- Drug and Alcohol usage (and if client is currently under the influence)
- Risk of harm to self/others
- Client’s transportation needs
- If you have a client who is not currently in crisis but who may present to Netcare in a crisis in the future, please complete and fax (614) 278-0135 -or place on the FTP server if you belong to a Lead Agency, a Netcare Crisis Alert so that we may provide the best care to your client should s/he arrive.
